Reactivating a State ID Number


 

To reactivate your STATE ID NUMBER, please do the following:

  • Download our form 1358 (Permanent State ID Number Application)
  • In the section titled "CHANGES TO STATUS OR INFORMATION FOR AN EXISTING STATE ID NUMBER", enter your current ID number. Then choose box 4. "I am reactivating this ID Number"
  • Complete the remainder of the form, in sections 6-12.
  • Sign and date the form in box 13.
 
Once it is completed, you can send it to us in one of 3 ways:

1. You can send this application by mail to:

                   PO Box 806
                   Sacramento, CA 95812-0806

2. You may also scan the application into PDF and email it to: idnumber@dtsc.ca.gov
3. You may fax the application to 916-324-6799 (No confirmation of receipt is sent for faxes
