Inactivate A Hazardous Waste EPA ID Number
Permanent State EPA ID Number Inactivation
A permanent State EPA ID number can be inactivated in one of the following ways:
- Complete and submit
DTSC Form 1358 – Permanent State ID Number Application.
- Inactivate through the electronic Verification Questionnaire (eVQ) System.
- Inactivate through a Letter of Inactivation.
Permanent Federal EPA ID Number Inactivation
A permanent federal EPA ID number can be inactivated in one of the following ways:
- Complete and submit EPA 8700-12.
- Inactivate through the electronic Verification Questionnaire (eVQ) System.
- Inactivate through a Letter of Inactivation.
Inactivate Through a Letter of Inactivation
Send a “Letter of Inactivation” on company letterhead to DTSC. The letter should contain the following information:
- The EPA ID number to be inactivated
- The site address the EPA ID number is registered to
- A current contact address and phone number
- An original “wet signature” from the owner or authorized employee and the date of when it was signed

Inactivate an ID Number Through the Electronic Verification Questionnaire (eVQ) System
eVQ Cycle Open. Both permanent State and federal EPA ID numbers may be inactivated in the eVQ System when the eVQ cycle is open. To inactivate a permanent EPA ID number in the eVQ System please follow one of the following instructions:
I Have An eVQ Account
- Go to the eVQ homepage. Click on the “Log Into eVQ System” button and sign in with your username and password.
- Complete Steps 1-2.
- In Step 3, under your EPA ID number, check the “Inactivate” box, and save the changes to your record.
- Proceed with Steps 4-5 to complete the questionnaire.
- No Fees Owed. Your EPA ID number will be inactivated within three (3) business days.
Fees Owed. Before your EPA ID number will be inactivated, you will need to pay for your fees first, then your fees will need to be posted by DTSC’s Accounting Unit. Payments may take between 10-15 business days to post from the date DTSC’s Accounting Unit receives your payment.
I Do Not Have An eVQ Account
- Go to the eVQ homepage. Click on “Request New eVQ Account”. Complete all the required fields to create an account.
- After you have successfully created your account, go back to the eVQ homepage and click on “Log Into eVQ System.” Sign in with your username and password.
- Complete Steps 1-2.
- In Step 3, under your EPA ID number check the box “Inactivate EPA ID”.
- Proceed with Steps 4-5 to complete the questionnaire.
- No Fees Owed. Your EPA ID number will be inactivated within three (3) business days.
Fees Owed. Before your EPA ID number will be inactivated you will need to pay for your fees first, then your fees will need to be posted by DTSC’s Accounting Unit. Payments may take between 10-15 business days to post from the date the Accounting Unit receives your payment.
eVQ Cycle Closed. If the eVQ cycle is closed, please read the information under the respective headings above “Permanent State EPA ID Number Inactivation” or “Permanent Federal EPA ID Number Inactivation” to inactivate your EPA ID number.
Temporary ID Number Inactivation
Since temporary State and federal EPA ID numbers expire in 90 days from the date they are issued they are not inactivated.
