Reactivate A Hazardous Waste EPA ID Number

Permanent State EPA ID Number Reactivation

A permanent State ID number may be reactivated in one of the following ways:

  1. By completing and submitting DTSC Form 1358 – Permanent State ID Number Application.
  2. Through the electronic Verification Questionnaire (eVQ) System.

Permanent Federal EPA ID Number Reactivation

To reactivate your permanent federal EPA ID number you may do so in one of the following ways:

Electronically

To submit US EPA Form 8700-12 electronically, you will need to have a myRCRAid account. MyRCRAid is a module of US EPA’s RCRAInfo System.

I do not have a myRCRAid account.

  • Register as an Industry User for a myRCRAid account through the RCRAInfo System.
  • After you have successfully created an account, you will then be able to complete US EPA Form 8700-12 electronically.
  • For more information on how to register for a myRCRAid account and instructions on completing US EPA Form 8700-12 electronically, visit DTSC’s Using myRCRAid page.
