Reactivate A Hazardous Waste EPA ID Number

Permanent State EPA ID Number Reactivation

Inactive Date of June 29, 2017 or Before. To reactivate your permanent State EPA ID number complete and submit DTSC Form 1358 – Permanent State ID Number Application.

Inactive Date of June 30, 2017 or After. To reactivate your permanent State EPA ID number, go to the eVQ homepage to complete the 2018 electronic Verification Questionnaire (eVQ). For more information, read the section titled “Reactivate an EPA ID Number Through the eVQ System” located on this page.

Permanent Federal EPA ID Number Reactivation

Inactive Date of June 29, 2017 or Before. To reactivate your permanent federal EPA ID number you will need to complete and submit US EPA Form 8700-12 – RCRA Subtitle C Site Identification Form. You have two options to submit the form. We recommend submitting the form electronically for a faster processing time. I want to submit US EPA Form 8700-12

To submit US EPA Form 8700-12 electronically you will need to have a myRCRAid account. MyRCRAid is a module of the RCRAInfo System created by US EPA.

I do not have a myRCRAid account.

  • Register as an Industry User for a myRCRAid account through the myRCRAInfo System.
  • After you have successfully created an account you will then be able to complete US EPA Form 8700-12 electronically.
  • For more detailed information on how to register for a myRCRAid account and instructions on completing US EPA Form 8700-12 electronically visit DTSC’s Using myRCRAid page.

I have a myRCRAid account.

The processing time for electronic submissions are 7-10 business days from the date DTSC verifies that your application is complete.

By Mail

Download  US EPA Form 8700-12. The application begins on page 113. Photocopies are not accepted. Mail the completed application with original “wet” signatures to:

Reactivate an EPA ID Number Through the Electronic Verification Questionnaire (eVQ) System

eVQ Cycle Open. Both State and federal EPA ID numbers may be reactivated in Step 3 of the eVQ when the eVQ cycle is open. To reactivate an EPA ID number in the eVQ System please select one of the following instructions:

I Have An eVQ Account

  1. Go to the eVQ homepage. Click on “Log Into eVQ System” and sign in with your username and password.
  2. Complete Steps 1-2.
  3. In Step 3, under your EPA ID number check the box “Reactivate EPA ID” and save your changes.
  4. Proceed with Steps 4-5 to complete the questionnaire.
  5. No Fees Owed. Your EPA ID number will be reactivated within three (3) business days.
    Fees Owed. Before your EPA ID number will be reactivated you will need to pay for your fees first, then your fees will need to be posted by DTSC’s Accounting Unit. Payments may take between 10-15 business days to post from the date the Accounting Unit receives your payment.

I Do Not Have An eVQ Account

  1. Go to the eVQ homepage. Click on “Request New eVQ Account”. Complete all the required fields to create an account.
  2. After you have successfully created your account, go back to the eVQ homepage and click on “Log Into eVQ System.” Sign in with your username and password.
  3. Complete Steps 1-2.
  4. In Step 3, under your EPA ID number check the box “Reactivate EPA ID” and save the changes.
  5. Continue with Steps 4-5 to complete the questionnaire.
  6. No Fees Owed. Your EPA ID number will be reactivated within three (3) business days.
    Fees Owed. Before your EPA ID number will be reactivated you will need to pay for your fees first, then your fees will need to be posted by DTSC’s Accounting Unit. Payments may take between 10-15 business days to post from the date the Accounting Unit receives your payment.

Most Recent Year’s eVQ Completed. If you have already completed the most recent year’s eVQ you will not be able to access the questionnaire. Please call 800-612-6942 to have your eVQ reset so that you can access the questionnaire. You will need to go through each step again and in Step 3, check the box next to “Reactivate EPA ID” to reactivate your EPA ID number. Continue with Step 4 and sign the questionnaire electronically again in Step 5.

eVQ Cycle Closed. If the eVQ cycle is closed, please read the information under the respective headings above “Permanent State EPA ID Number Reactivation” or “Permanent Federal EPA ID Number Reactivation” to reactivate your EPA ID number.

Temporary EPA ID Number Reactivation

Temporary EPA ID numbers cannot be reactivated. If your temporary State or federal EPA ID number has expired or will be expiring and you still need use of an EPA ID number, you are encouraged to apply for either a permanent State EPA ID number or a permanent federal EPA ID number.
