Reactivate A Hazardous Waste EPA ID Number

Permanent State EPA ID Number Reactivation

A permanent State ID number may be reactivated in one of the following ways:

  1. By completing and submitting the  DTSC Form 1358 – Permanent State ID Number Application.
  2. Through the electronic Verification Questionnaire (eVQ) System.

Permanent Federal EPA ID Number Reactivation

To reactivate your permanent federal EPA ID number you may do so in one of the following ways:

  1. Through the electronic Verification Questionnaire (eVQ) System
  2. By completing and submitting the U.S. EPA Form 8700-12 – RCRA Subtitle C Site Identification Form. You may submit the form electronically or by mail. We recommend submitting the form electronically for faster processing.

Electronically

To submit the U.S. EPA Form 8700-12 electronically, you will need to have a myRCRAid account. MyRCRAid is a module of the U.S. EPA’s RCRAInfo System.

I do not have a myRCRAid account.

  • Register as an industry user for a myRCRAid account through the RCRAInfo System.
  • Log into your account to access the U.S. EPA Form 8700-12 electronically.
  • For more information on how to register for a myRCRAid account and instructions on completing U.S. EPA Form 8700-12 electronically, visit DTSC’s Using myRCRAid page.

I have a myRCRAid account.

The processing time for an electronic submission is 7-10 business days from the date DTSC verifies that your application is complete.

By Mail

Download  US EPA Form 8700-12. The application begins on page 113. Only applications with original “wet” signatures will be accepted. Please mail the completed application to the following address:

The processing time for paper submissions is 10-15 business days, from the date DTSC verifies that your application is complete.

Reactivate an EPA ID Number Through the Electronic Verification Questionnaire (eVQ) System

eVQ Cycle Open. Both State and federal EPA ID numbers may be reactivated when the eVQ cycle is open. To reactivate an EPA ID number in the eVQ System, please select one of the following sets of instructions:

I Have An eVQ Account

  1. Go to the eVQ homepage. Click on “Log In” and sign in with your username and password.
  2. Complete steps 1-2.
  3. In step 3, under your EPA ID number, check the “Reactivate” checkbox and save the changes to the record.
  4. Proceed with steps 4-5 to complete the questionnaire.
  5. No Fees Owed. Your EPA ID number will be reactivated within three business days.
    Fees Owed. Before your EPA ID number will be reactivated, you will need to pay for your fees first. Then, your fees will need to be posted by DTSC’s Accounting Unit. Payments may take between 10-15 business days to post from the date DTSC’s Accounting Unit receives your payment.

I Do Not Have An eVQ Account

  1. Go to the eVQ homepage. Click on “Request New eVQ Account”. Complete all the required fields to create an account.
  2. After you have successfully created your account, go back to the eVQ homepage, and click on “Log Into eVQ System.” Sign in with your username and password.
  3. Complete steps 1-2.
  4. In step 3, under your EPA ID number, check the “Reactivate” box and save the changes to the record.
  5. Continue with steps 4-5 to complete the questionnaire.
  6. No Fees Owed. Your EPA ID number will be reactivated within three business days.
    Fees Owed. Before your EPA ID number can be reactivated, you will need to pay for your fees first. Then, your fees will need to be posted by DTSC’s Accounting Unit. Payments may take between 10-15 business days to post, from the date DTSC’s Accounting Unit receives your payment.

eVQ Cycle Closed. If the eVQ cycle has closed, please submit the DTSC Form 1358 to reactivate a permanent State EPA ID number or the EPA Form 8700-12 to reactivate a federal EPA ID number. 

Temporary EPA ID Number Reactivation

Temporary EPA ID numbers cannot be reactivated. If your temporary State or federal EPA ID number has expired or will be expiring and you still need to use an EPA ID number, depending on the type of waste and the amounts you are generating, you will apply for either a permanent State EPA ID number or a permanent federal EPA ID number.
